Please refrain from using email for sensitive health details; contact us by phone or schedule an appointment instead.
We encourage the use of the Spruce App for communication with our practice and providers whenever possible.
The Spruce App facilitates faster and more secure messaging compared to email.
Utilizing the app is the best way to ensure the privacy of your health information in electronic correspondence with our office.
In case of urgent issues, please call our office directly.
For non-urgent matters, the Spruce App is the preferred method of digital communication.
This approach helps protect your confidentiality and allows us to respond promptly.
One full business day (24 hours prior, excluding weekends) notice is required for appointment cancellations.
Please be aware that appointments not canceled or rescheduled within 24 hours will incur a $100 no-show fee, charged automatically to the credit card on file.
Future appointments will not be scheduled until the fee is paid.
Late cancellations or no-shows impact others seeking treatment and create a financial burden for our practice.
We appreciate your understanding of the necessity of this policy.
Multiple reminders are sent through various channels to notify you of upcoming appointments.
To cancel or reschedule, provide at least 24-hour notice (weekends not included).
For Monday appointments, cancellations must be made by the same hour on the preceding Friday.
Video/online appointments are available.
If you miss your appointment, refills will not be provided until you see your provider.
For disability form completion, our small practice with limited staff follows this policy:
Long-term disability or SSI forms require a client’s six-month history with our office; medical records can be provided, but eligibility determination is not within our scope.
Short-term or FMLA forms necessitate two completed visits.
Paperwork may take up to fourteen (14) days, processed on a first-come-first-served basis. Rushing is challenging due to limited staff.
Compliance with ongoing treatment, including medications and appointments, is required for continued form filling.
Clients must have seen the provider within 30 days to submit disability paperwork.
Blank forms are not accepted; at a minimum, client’s name and date of birth must be filled out.
A fee of $50-$150 per form is due before the form is returned to the client.
- As per your insurance plan, deductibles or co-payments are expected at the time of service.
- You are responsible for any remaining balance if your insurance does not fully cover charges.
- Verify that this practice accepts your insurance plan before your visit.
- Bring your insurance card and provide complete information before the initial appointment for coverage and benefits verification.
- Some plans may require pre-authorization for specific services, and obtaining authorizations is the patient’s responsibility.
- If pre-authorization is necessary but not obtained, you may be financially responsible for uncovered charges.
- Contact your insurance carrier in advance for pre-authorization details and coverage understanding for planned services.
- Our staff is available to assist with benefit verification and required authorizations.
Prescriptions for controlled substances are issued solely during regularly scheduled appointments with the prescribing provider.
Lost, stolen, or misplaced prescriptions will not be replaced under any circumstances.
No early refills are authorized for controlled substance prescriptions; refills must align with the original prescription and be requested during a scheduled office visit.
New patients prescribed controlled substances must undergo a urine drug test before the first prescription, agreeing to random urine drug screens.
Established patients on controlled substances require at least an annual urine drug test; additional, random testing may occur at the provider’s discretion based on individual risk factors.
Benzodiazepines pose risks of dependency and misuse, intended for short-term use in treating acute conditions per FDA guidelines.
Long-term, daily use of benzodiazepines requires significant documented need and entails a tapering plan for discontinuation after appropriate treatment duration.
You have the right to receive a cost estimate for your medical care, as mandated by federal law.
Healthcare providers are obligated to furnish uninsured or self-pay patients with a Good Faith Estimate for non-emergency medical items and services.
The estimate encompasses anticipated costs for related items like diagnostic tests, prescriptions, equipment, and hospital fees.
Request the Good Faith Estimate in writing from your healthcare provider at least one business day before any scheduled medical procedures or treatments.
You can ask any provider for a fee estimate before receiving care, ensuring transparency into your financial responsibility for healthcare.
